April 16th is National Advance Care Planning Day. Advance Care Planning is a process of thinking and documenting your values and wishes for health care if you were sick, or injured and could not speak for yourself. In light of the COVID-19 pandemic, it’s now more important than ever to give consideration to your heath care directives and discuss them with your loved ones.

End of Life Doulas Jennifer Mallmes and Janine Jordison share the five steps of advance care planning and explore the value an End of Life Doula can add when facilitating an advance care plan. Get prepared with the tools you need to start the conversation on National Advance Care Planning Day.
